Your application will be considered complete when we have received each required item listed below:

Fill out and submit the online application. There is no fee for applying. You will receive a confirmation email (with a copy of your application) directly after your submission.
✔ Teacher recommendation forms
- For All Applicants:
Download and print the Teacher Recommendation form (Required). Fill out the top section and ask your child’s current (or most recent) teacher to complete the form and mail it to us within a week of your application date. To expedite the process, we suggest providing your recommender with a stamped envelope made out to Chewonki Elementary & Middle School Admissions at the address below.
- For Students Applying to 5th – 8th Grade:
Supplemental Math Teacher Recommendation Form (Required)
Supplemental English Literacy Recommendation Form (Required)
✔ Student records (required)
Download, print, and fill out the Request for Copy of School Records form and give it to your current (or most recent) school administrator along with a stamped envelope made out to Chewonki Elementary and Middle School Admissions at the address below. Please request that a copy of your child’s records get sent to Chewonki within a week to ensure that your application is not delayed.
✔ Student Work Samples (required)
Since we are unable to host student applicants on campus at this time, it would be helpful for us to get a sense of some of the academic school work your child has been doing this spring. Download the student work sample sheet here.
✔ Financial aid application (optional)
Families intending to apply for financial aid from Chewonki should fill out a Parents’ Financial Statement (PFS) at sssbynais.org/parents. The non-refundable fee is approximately $52 for the application and you will receive a confirmation email directly after your submission.

The priority deadline for the 2022-23 school year is January 31, 2022. All applications received by that date will be reviewed together. If there are spots remaining after the initial admissions deadline, applications will then be considered on a rolling basis. If your application was submitted by the January 31 deadline, you will receive an admissions decision by March 1. If you are being considered on a rolling basis, you will receive an admissions decision approximately 2 weeks after submitting your completed application.
