Welcome from the Chewonki Health and Wellness Center (HWC). The center is staffed by a team of registered nurses and a Mental Health Coordinator that care for the Elementary Middle School students, as well as the more than 1,000 other participants that Chewonki hosts around the year. One of the HWC team is appointed to be the lead nurse for Chewonki Elementary Middle School. This nurse is the contact for your families and will be responsible for ensuring the school is following the Department of Education health policies. If you need to email the HWC it is best to use Health@Chewonki.org instead of an individual nurse’s email address. Please enter “EMS” in the subject line. The Chewonki Health and Wellness Center utilizes Schooldoc (app.schooldoc.com) for their electronic health records. You will receive an invitation to the site when your application is accepted. There is an open period for completing the Health Profile. If you need to get back in to make changes please contact the HWC and staff can unlock it for you.
The Health Form in Schooldoc is complete and thorough. A place-based education program like ours comes with inherent demands. It is necessary for the Health and Wellness team to get a true representation of each student’s physical and mental health to ensure a successful educational experience. We strive to set students up for success so transparency is a necessity.
The Health Profile in Schooldoc consists of:
- Contact Information: This includes emergency contacts
- Dietary preferences/restrictions: Gluten-free and Dairy-free are two of the options
- Allergies: Questions about food allergies and other allergies
- Emergency Action Plans: to be signed by a provider
- Medications:
- Please list any daily medications your student is taking
- Enter emergency or as-needed medications like EpiPens and inhalers
- Permission to self-administer form to be signed by a provider
- Psychotropic medication policy to be signed by the prescriber
- Over-the-counter medications:
- Your authorization is needed for the HWC staff to administer these medications during the school day.
- Health History: This includes questions about chronic illnesses and recent injuries.
- Mental Health: A complete profile for the Mental Health Coordinator.
- Immunizations: This record can be downloaded from a patient portal and uploaded directly into Schooldoc. Please attach a note if the student is on an immunization catch-up plan. New (2021) state law does not allow for philosophical or religious exemptions. For a medical exemption, a form must be completed by your provider.
- Insurance: Please provide pictures of your insurance card, back and front.
- Healthcare Providers: Contact information of student’s medical and mental health providers.
- Healthcare Provider Form: This form must be signed by a provider before the start of the school year as it clears the student for activities while at school. The form can be completed using information from a visit within a year of the starting date.
- Authorization: This must be signed before the start of school as it gives permission for the HWC staff to treat the student if they become ill or injured while at school.
